Job Summary

Children’s Hospital of Philadelphia (CHOP) is seeking a Resource Mapping Coordinator to join our interdisciplinary team focused on community-clinical partnerships. The Resource Mapping Coordinator will work in partnership with the Division of Social Work to help advance family-centered care and the mission of Healthier Together, a 5-year initiative to improve the health of children in West Philadelphia.


The Resource Mapping Coordinator will play a critical role in helping maintain, update and promote engagement for the CHOP community resource map. This role will also provide support for a community health worker program based under the Division of Social Work.


This position is ideal for a candidate interested in social work or public health, or a community health professional who is looking to utilize their technical skills to support clinical teams and patients in connecting with services. The ideal candidate is an organized and driven team player, with the ability to problem-solve independently and exhibit professionalism in multiple interdisciplinary settings.

Job Responsibilities

Support the coordination of efforts to build and grow a community resource map (75% FTE)


  • Partner with the CHOP Community Resource Mapping team to help maintain and update the CHOP community resource map and database. 
  • Serve as a point of contact for CHOP staff with questions around or updates for the CHOP resource map.
  • Gather and communicate feedback from CHOP clinical teams, community organizations and patients to help inform the design of the resource mapping website. 
  • Assist with training new clinical teams and community organizations in using the CHOP resource map. 


Programmatic Work (25% FTE)


  • Partner with the Division of Social Work’s clinical social work supervisor to provide administrative support for the social work Community Health Worker program.
  • Help organize professional development and training opportunities for community health workers, utilizing both internal resources and external partnership opportunities. 


The position requires collaborative, interpersonal, organizational, and communication skills as well as proficiency in database entry and management.

Required Education and Experience

Required Education: Bachelor’s Degree


Required Experience: At least one (1) year experience related to social work, public health, or non-profit leadership experience required.

Preferred Education, Experience & Cert/Lic

Preferred Education: Bachelors in Social Work

  • Experience with technology and ability to learn new technologies quickly. Proficiency with data entry and database management.

  • Demonstrated experience and content expertise in family-centered care or community
