Job Summary

The Neurology Surgical Nurse Navigator will provide guidance, oversight, and coordination for the following activities:

  • Represents patients and families coming to CHOP from local, regional, national, and international sources including referring physicians, hospitals, community at-large and other ancillary agencies by coordinating their care.
  • Works independently to develop patient, family and provider relationships and achieve targeted goals.
  • Coordinates patient care services and maintains interpersonal relationships with patients, families and referral sources, from initial contact through discharge and follow-up. 
  • Works with medical and administrative leaders to assist in the development and implementation of educational and outreach activities for the Program.

 

Job Responsibilities

  • Coordinates referrals and collection of appropriate medical records and diagnostic information; abstracts pertinent clinical information as necessary.
  • Acts as the primary point of contact for patients/families and referring providers ensuring ease of access and ongoing communication, continually striving to enhance the CHOP experience, focusing on family centered care.
  • Maintains a relationship with the patient/family and referring provider throughout treatment course (evaluation, treatment and follow up phases of care), providing updates and relaying pertinent medical information.
  • Provides instruction in disease process, treatment plan, health and skill teaching.
  • Assesses health status and identifies health care needs.
  • Assess patient/family’s psychosocial and support needs, initiating appropriate referrals.
  • Supervises scheduling processes by assisting in triaging new patients for urgency; assigning new patients/consults to appropriate providers; and ensuring coordinated care for patients in need of multiple provider and diagnostic appointments.
  • Participate in appropriate meetings relevant to patient’s management, i.e. case conference, etc.

Outreach and Program Development

  • Develops and implements strategies to increase utilization of Program’s services and physicians.
  • Identifies potential referring physicians and develops a strategy to encourage them to become CHOP referring physicians and works to maintain ongoing relationships.
  • Periodically develops, refines and updates outreach materials, such as survey tools, fact sheets, and patient/family/provider education.
  • Participates in philanthropic activities that support programmatic initiatives.
  • Works with the Web and Marketing Teams to assist in the development of program web and print content, including patient education materials.

Resource Utilization and Collaboration

  • Collaborates with all appropriate CHOP departments to strengthen relationships and facilitate the patient’s pre-surgical process.
  • Meets routinely with strategic internal partners to ensure consistent messaging and follow through.

Data Management

  • Works with collaborators to develop and maintain survey tracking tool for referrals
  • Captures, tracks and develops reporting mechanisms on key indicators
  • Provides referral demographics (reports) and analyses to support departmental growth objectives.

Leadership

  • Acts as a resource and mentor for staff (i.e. administrative staff, nursing) regarding patient or family processes or procedure inquiries, and complex patient issues.
  • Continually evaluates communication processes with all stake holders (i.e. referring physicians, hospitals, CHOP Faculty and staff, families/patients, etc.) and recommends improvement strategies if needed.

Required Licenses, Certifications, Registrations

Active RN license required

Required Education and Experience

Required Education:  Nursing Diploma.

 

Required Experience: At least three (3) years of pediatric nursing experience. Experience in a fast-paced and complex pediatric healthcare environment.

Preferred Education, Experience & Cert/Lic

Preferred Education: Bachelor’s degree

 

Preferred Experience: Experience in pediatric neurology/neurosurgical pateint care.

Additional Technical Requirements

  •  Expertise in pediatric nursing/ with knowledge of neurology nursing.
  •  Proficient in Microsoft Word and Excel.
  •  Ability to complete multiple tasks and work under pressure.
  •  Able to adhere to strict deadlines.
  •  Excellent Customer Service skills, phone etiquette, and communication skills.
  •  Ability to work independently.
  •  Excellent organizational and documentation skills.
  •  Ability to handle confidential materials and information.
  •  Demonstrate professionalism and a friendly manner with patients, families and other staff.
  • Ability to travel on a local, regional and national level.
