Job Summary

The Cardiac Care Coordination Counselor will provide guidance, oversight and coordination to the following activities:

  • Serves as a resource to families and providers to facilitate communication and coordination across multiple inpatient and outpatient clinical services.
  • Provides education and guidance for families to effectively promote complex care coordination within the Cardiac Center and other CHOP Clinical areas as needed.
  • Provides tools to support family and provider access to clinical information.
  • Interprets requests, identifies gaps, and communicates with families and/or referring physicians to gather information as needed to effectively coordinate plans for scheduling inpatient and outpatient including Cardiac Surgery, Cardiac Catheterization Diagnostic and Interventional Procedures, Electrophysiology, Cardiac MRI, Lymphatic Imaging and Intervention procedures and other clinical care in a way that minimizes patient/family anxiety and promotes positive patient family satisfaction. 
  • Assesses family/patient needs and triages to appropriate clinician for appropriate clinical follow-up.
  • Partners to build a network of key contacts/experts across the network to support care coordination activities.
  • Coordinates scheduling of complex services for inpatients and outpatients.
  • Provides clinical guidance to medical schedulers to promote accurate and timely access to services.
  • Provides timely and accurate responses to all inquiries regarding status of intake/referral.

Job Responsibilities

Coordination of Care

  • Ensures proper coordination and balance between inpatient and outpatient elective schedules and inpatient urgent procedural needs.  Multi-tasking is essential to simultaneously facilitate multiple complex cardiac and non-cardiac services.
  • Identifies procedural options for all services and discusses plan with Cardiac Center resources when needed.
  • Works closely with all Cardiac Access Center customers, including but not limited to The Cardiac Referral Program, Intake Center, Cardiology, Lymphatic Program, Cardiac Anesthesia, Adult Congenital Heart Disease Program, Fetal Heart Program, Cardiology Research Group, CT Surgery staff, PARC and associated hospital staff to minimize turnaround time, optimize patient flow and improve family and practitioner experience through streamlined and appropriate access to all CHOP services.
  • Proactively identifies implications of the current inpatient population, upcoming Special Delivery Unit and Fetal Heart Program activity, as well as current hospital and Cardiac Center census, to optimize planning for elective procedure scheduling opportunities.  Involves Cardiac Center 21800 inpatient Bed Team as needed.
  • Promotes integration of services between the outpatient scheduling team and procedural teams.
  • Educates Cardiac Center team members on access opportunities throughout the center.
  • Supports Cardiac Referral Program, and CHOP international initiatives, to implement identified plan of care.
  • Advocates for every patient and family to coordinate all needs, demonstrating patient and family focused problem solving skills.  Assists families with navigating the complex CHOP network to coordinate appointments with other specialties. Assists families who may have barriers to services.
  • Notifies families, staff and appropriate referral source of scheduled appointments/plan.
  • Provides financial counseling guidance requiring clinical expertise.
  • Discusses changes, need for postponing/rescheduling scenarios with families and requestors, and implements new plans. 


Utilization of Resources

  • Tracks metrics identified by Cardiac Access Manager; Implements improvement efforts as needed.
  • Understands all equipment resource needs of the Cardiac Operative and Imaging Complex and develops/implements systems to properly plan optimal utilization of these resources while maintaining the correct match between patient needs and services provided. 
  • Monitors the match between provider availability and capacity of elective services to promote a balanced schedule.
  • Promotes inpatient throughput by properly planning cases associated with trends in patient population, length of stay, day of week, etc.

  • Schedules all appointments as necessary.  Documents accurately and concisely in multiple applications:
  • Gathers and updates all appropriate registration and clinical data as needed.
  • Distributes information for review and assessment to the appropriate cardiac center staff.
  • Documents resolution/outcome of family/referring physician follow-up.
  • Publishes workflow and volume status reports as developed by CAC Manager and Cardiac Center administration.
  • Participates in the development and monitoring of Performance and Process Improvement activities.
  • Makes suggestions for new ideas for performance improvement tools.



  • Serves as a resource person for other Cardiac Center staff to access when questions arise regarding intake information, scheduling conflicts, Epic information/troubleshooting and difficult patient access issues.
  • Mentors Cardiac Access team members through education and feedback opportunities. 
  • Supports goals of The Cardiac Center.
  • Provides coverage for Cardiac Access Manager and Cardiac Referral Program as needed.

Required Licenses, Certifications, Registrations

RN - Registered Nurse

Required Education and Experience

Required Education: Graduation from an ACEN and/or CCNE accredited school of nursing or college


Required Experience: At least three (3) years of prior cardiac nursing experience


Preferred Education, Experience & Cert/Lic

Preferred Education: BSN


Preferred Experience: Five (5) or more years of nursing experience.

Additional Technical Requirements

  • Proficient in Word, Excel, Outlook.
  • Ability to complete multiple tasks and work under pressure.
  • Able to adhere to strict deadlines.
  • Proficiency with Epic, CardioIMS application a plus.
  • Excellent Customer Service skills, phone etiquette, and communication skills.
  • Ability to work independently.
  • Ability to demonstrate critical thinking skills through problem solving.
  • Excellent organizational and documentation skills.
  • Ability to handle confidential materials and information.
