Job Summary

The Pediatric Palliative Care RN will provide skilled assessment and intervention for children with chronic, complex and/or life-threatening conditions and their families to help determine available and appropriate resources across continuum of care. S/he will provide clinical consultation as an active member of interdisciplinary care team and attend to the system dynamics of the health care delivery network to foster best use of resources, effective service delivery and patient and family satisfaction as well as ensure compliance with practice standards and codes.

Job Responsibilities
  • Participates in an Interdisciplinary approach with the Palliative Care Team in the evaluation of end of life care for patients and families.
  • Collaborates with primary specialty teams and patients and families in identifying the most appropriate and safe discharge plan, needs and resources.
  • Coordinates discharge plan for home or inpatient hospice needs (end of life care).
  • Seeks/obtains appropriate insurance approval for end of life care services.
  • Advocates for patients/families with appropriate referral agencies.
  • Collaborates with Palliative Care Team to maintain clinical data base for palliative care patients/families.
  • Communicates and coordinates with outside agencies providing home care services for outpatients who are receiving palliative care.
  • Assists in maintaining documentation systems for end of life patients/families.
  • Communicates with staff members involved in end of life care via patient care rounds.
  • Collaborates with Palliative Care Team in establishing guidelines for patient care conferences, educational programming and in-service training on end of life care.
  • Serves as a resource to staff members who are caring for dying patients.

Required Licenses, Certifications, Registrations
Registered RN License (PA or NJ depending)
Required Education and Experience

 

Required Education: BSN Required. Graduate of an ACEN and/or CCNE accredited school of nursing or college.

Required Experience: 3 years’ nursing experience.

Preferred Education, Experience & Cert/Lic

 

Preferred Experience:

  • Pediatric care coordination/case management experience.
  • Demonstrated experience working with children and families in a clinical capacity.
  • Demonstrated experience working with life-threatening conditions, chronic disease, hospice and/or end of life care and related situations.

     

Additional Technical Requirements

 

Computer Competency

  • Basic use of a computer (mouse, keyboard, printer, USB ports).
  • Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype).
  • Web browsing, intranet search, document access.
  • Use of shared network file space.
  • Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors).
  • Navigation and use of clinical communication systems.

 

Information Literacy

  • Determines the nature and extent of clinical information needed and uses the appropriate technology to access it.
  • Accesses needed information effectively and efficiently.
  • Evaluates information and its sources critically and appropriately incorporates it into clinical practice.
  • Evaluates outcomes of the use of information.

     

Information Management Literacy

  • Navigation of the electronic health record system.
  • Ability to locate and review specific patient data in various clinical information systems.
  • Effectively uses clinical decision support tools.
  • Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.
  • Ability to collect clinical data using quality improvement tools.
