Job Summary

The Nurse Navigator for the Division of Plastic Surgery Craniofacial Team is responsible for collaborating with our multidisciplinary team to allow for timely diagnosis, treatment, and increased supportive care of our craniofacial patients and families.  The Craniofacial Team Nurse Navigator will be the point person for external referrals, must maintain a strong relationship with families throughout their course of care and seek to continually improve the care model for these patients.  In addition to direct patient and family interaction, the nurse navigator will partner with marketing and development to ensure we have a strong social media presence and design events for our craniofacial families that promote support and peer interactions in a fun, low stress environment.  The candidate should be comfortable with attendance and presentations at national meetings related to Craniofacial conditions and the care models provided to these patients.

Job Responsibilities

Referrals Coordination and Triage

  • Expedite and facilitate communication for patient consultation, referrals, second opinions, and transfers including local, regional, and national patients and referrals
  • Coordinate care with referring clinicians and referral patients to simplify access, improve communication and enhance the patient / family experience
  • Coordinate patients’ plan of care and serve as primary contact for referring physicians
  • Provide the patient and the referring physician with updates and medical information
  • Act as point person for referral base (patient / families / physicians) regarding services
  • Maintain a relationship with the patient and family throughout the course of their care
  • Collaborate internally to strengthen relationships and facilitate the referral process
  • Triage and resolve daily calls and messages 

 

Outreach

  • Develop and implement strategies to increase utilization of surgery services
  • Develop social media support group strategy with direct patient / family interaction
  • Support outreach groups including on-line forums and national / local organizations

 

Education and Care Coordination

  • Collaborate with advanced practice providers (APPs) and attending surgeons for complex operative patients
  • Collaborate to assure all specialists have submitted clearance letters
  • Provide pre/post-operative patient and family education and care coordination  
  • Coordinate housing, transportation and additional support for patients as needed
  • Participate in ambulatory appointments to assure ongoing care coordination

Required Licenses, Certifications, Registrations
Licensure as an RN in the Commonwealth of Pennsylvania required; NJ if applicable.
Required Education and Experience

Required Education:  Graduation from an ACEN and/or CCNE school of nursing or college.

 

Required Experience: Five (5) years of experience with surgical populations in both inpatient and ambulatory settings. 

Preferred Education, Experience & Cert/Lic

Preferred Experience: Experience working with patients/families with complex surgical needs  

Additional Technical Requirements
  • Advanced proficiency with surgical scheduling systems (Optime)
  • Advanced proficiency with medical database systems (REDCap)
  • Advanced proficiency with electronic medical record software (EPIC)
  • Intermediate proficiency with office software (Microsoft Office) including word processing, spreadsheet, and presentation software (Word, Excel, PowerPoint)
